What is the spending on Medicaid?

In FY 2024 the federal government and the states spent $829 billion on Medicaid.

Of the $829 billion total, the federal government contributed $567 billion and the states kicked in $262 billion.

Medicaid spending increased steadily from $325 billion in 2007 to nearly $500 billion in 2014. But then, with the implementation of the Patient Protection and Affordable Care Act of 2010, Medicaid began a three year rapid increase to $542 billion in 2015. Starting with the COVID year of 2020 Medicaid spending increased from $615 billion in 2019 to $838 billion in 2023.

In 2024 Medicaid spending was $829 billion.

Viewed from a GDP perspective, Medicaid spending increased from 2.25 percent GDP in 2007 to 2.65 percent of GDP in 2010 in the aftermath of the Great Recession. In 2014-15 Medicaid spending jumped up to 3 percent of GDP and has remained at about 3 percent of GDP into the 2020s.

In 2024 Medicaid spending was 2.8 percent GDP.

Medicaid, the joint federal/state health care program for the poor, was passed in 1965. Benefits began in 1966 and rapidly climbed to over 0.5 percent of GDP by 1971, before slowly flattening out to 1 percent of GDP in the late 1980s.

Starting in the 1990s Medicaid began a rapid increase in cost, hitting 1.5 percent of GDP in 1991, and 1.9 percent in 1995. In the recession of 2000-1 Medicaid spending took another bump up to 2.4 percent GDP in 2004, then up to 2.6 percent GDP after the Great Recession of 2006-08.

When the Patient Protection and Affordable Care of 2010 Act kicked in in the mid 2010s, Medicaid spending bumped up to 3 percent of GDP. In the COVID crisis of 2020 Medicaid bumped up to 3.3 percent GDP. In 2024 Medicaid spending was 2.8 percent GDP.

Medicaid, the joint federal/state health care program for the poor, was advertised as a 50-50 program, with 50 percent paid for by the federal government and 50 percent paid for by the states. This held until about 1990 when the federal share started increasing. By 2018, after the implementation of the PPACA Act, the federal share was 1.89 percent of GDP and the state share was 1 percent GDP.

In 2024 the federal Medicaid share was estimated at 1.93 percent GDP. and the state share was 0.89 percent GDP.

State and Local Finances for 2023

On September 11, 2025 we updated the state and local spending and revenue for FY 2023 using the new Census Bureau State and Local Government Finances summaries for FY 2023 released on July 31, 2025.  (See also Annual Survey of State and Local Government Finances). The release includes state and local spending for the United States as a whole and the 50 individual states and the District of Columbia.

State and local spending and revenue for FY2023 are now actual historical spending as reported by the Census Bureau. In addition, the Census Bureau published updated tables for 2021 and 2022.

We have updated the "guesstimated" state and local finances for FY2024-30 as indicated in our "guesstimate" blog entries.

We have also updated data for individual local government units with data for 2023. 

Beginning in 2022 the Census Bureau has changed the value for Line 56 Direct Expenditure and Line 7 General Revenue from own sources, as follows:

We have decided to end our publication of non-insurance trust cash and security holdings.

However, to keep the time series at usgovernmentspending.com consistent, we have decided to add insurance-trust values back into Line 56 and Line 7 values.
